Job Summary

We are seeking a skilled RN Case Manager to join our team at UF Health Shands. As a key member of our healthcare team, you will be responsible for assessing, planning, implementing, and monitoring comprehensive care plans for patients.

Key Responsibilities

• Collaborate with healthcare professionals to develop and implement patient care plans
• Conduct thorough assessments of patients' medical and social needs
• Develop and implement strategies to improve patient outcomes and reduce hospital readmissions
• Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
• Stay up-to-date with the latest clinical guidelines and best practices in case management

Requirements

• Current RN licensure in the State of Florida
• 2 years of recent RN experience in a clinical capacity
• Prior case management experience preferred
• BSN or MSN degree preferred
• Professional nursing board certification in specialty area preferred
