    Job Summary and Qualifications

    The Registered Nurse in Admission and/or Discharge coordinates and delivers high quality, patient-centered care by overseeing the effective management of patients into and out of the acute care hospital setting. The RN Admission/Discharge utilizes specialized assessment skills to ensure quality admission to the appropriate care areas, and also safe discharges of acute care patients requiring education and post hospitalization care. The RN Admission/Discharge serves as an advocate for patients/families/caregivers and models a commitment to the organization’s vision/mission/values to support an unparalleled patient experience and optimum clinical outcomes.

    What you will do in this role:

    Performs direct patient carein accordance with the nature and specific requirements of the facility, and consistent with the scope and standards of practice for the relevant state.

    Receives physician orders for direct admissions and organizes admission process for patient.

    Obtains a nursing assessment history, and insures proper inpatient care unit admission. Takes appropriate additional action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.

    Completes and facilitates the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.

    Teaches patients/families/caregivers about patient medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete understanding.

    Teaches patients/families/caregivers about any non-medicinal follow-up measures, such as healthy diet and exercise, disease prevention, and/or other lifestyle changes. Prepares patients and families/caregivers for future self-management.

    Completes additional discharge documentation, education, and discharge care for special patient populations,to ensure compliance with core measures for discharge.

    Ensures patients patients/families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.

    Manages the continuation of care by serving as the interface between healthcare providers and community support systems who coordinate ongoing care needs for specialized patient groups.

    Establishes and maintains communication process with all patient care; and for smooth operations of admitting, discharge and transfer. Gathers data relevant to the patient’s individual needs and age group.

    Coordinates with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.

    Demonstrates a time-conscious awareness and ability to prioritize. Distinguishes between low and high priority activities.

    Seeks creative alternatives and innovative possibilities in problem solving. Demonstrates a strong ability to identify, analyze and solve problems and can make sound decisions. Understands and considers the ramifications decisions may have on other departments.

    Performs other duties as assigned.

    Practices and adheres to the “Code of Conduct” and “Mission and Value Statement.”

    What qualifications you will need:

    Must be licensed as a Florida Registered Nurse in accordance with state regulations or appropriate compact licensure. If compact license held, active FL RN license required within 90 days of hire.

    2+ Years applicable experience in an acute care setting

    ·Nursing Diploma or ASN required; BSN preferred.

    Current American Heart Association BLS (Basic Life Support) certification required.

    Current American Heart Association ACLS (Advanced Cardiac Life Support) certification required (6 months grace period given from hire date).

    Current NIHSS (National Institute of Health Stroke Scale) required (6 months grace period given from hire date).

    Critical Care Registered Nurse Acute/Adult (CCRN), or Certified Orthopedic Nurse (ONC), or Progressive Care Nursing Administrator (PCCN), or Board Certified Cardiac / Vascular Nurse, or Board Certified Medical Surgical Registered Nurse, or Certified Medical-Surgical Registered Nurse (CMSRN)

    Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills
