The Center for Excellence in Primary Care (CEPC) housed within the Department of Family and Community Medicine (DFCM) at Zuckerberg San Francisco General Hospital (ZSFG) is seeking an exceptional Trainer and Communications Coordinator who is passionate about improving healthcare delivery, particularly for the underserved. As a Trainer and Communications Coordinator at CEPC, you would share in our mission to identify, develop, test, and spread promising innovations in primary care to improve the patient experience, enhance population health and health equity, reduce the cost of care, and restore joy and satisfaction in the practice of primary care. Primary care transformation requires the implementation of essential components of high performing primary care, such as team-based care and data-driven improvement. CEPC calls these the Building Blocks of High Performing Primary Care. Drawn from the experiences of high performing primary care practice, the building blocks provide a roadmap to improvements in patient experience, population health, and the sustainability of primary care. CEPC has a unique opportunity to help primary care health centers, both within the safety net for San Francisco and more broadly across the country, integrate these Building Blocks into their practices through training to clinics seeking to build teams to meet the needs of their population. As a Trainer, this staff member will co-facilitate trainings and learning sessions alongside CEPC master trainers on CEPC’s standard curricula, which includes health coaching, panel management, and practice coaching. Health coaching is a patient-centered approach to delivering care that helps patients build the knowledge, skills, and confidence required to manage their chronic conditions and improve their health. Panel management is a proactive way to ensure that everyone assigned to a clinic is up to date on basic preventive care. Practice Coaching is a way to assist primary care practices and clinics to improve the way they deliver care. The Trainer and Communications Coordinator will also assist with development of new curricular materials, revision of existing materials, and developing, revising and tailoring training materials. As a Communications Coordinator for CEPC’s Training programs, this staff member will provide support for a broader host of CEPC training services by managing recruitment, registration, evaluation, logistics, contracts, vendor relations, customer service, and travel planning and reimbursement for the training team. This staff member will coordinate communications for the CEPC website, newsletters, and social media. The Trainer and Communications Coordinator will be involved in strategic planning for training programs and development of communication strategies via social media, website, and listservs. They will also provide support for other CEPC programs as needed. The final salary and offer components are subject to additional approvals based on UC policy. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement. The salary range for this position is $61,200 - $130,600 (Annual Rate). Demonstrated skills in designing trainings that apply adult learning theories and education methodologies. Demonstrated knowledge of the CEPC’s model of practice improvement – the 10 Building Blocks of High Performing Primary Care. Technical skills to manage and distribute electronic information across a variety of platforms. Working knowledge of functional area and understanding of how training may impact other areas. Good skills in delivering training programs. Good verbal, written and interpersonal communication skills and good presentation skills. Preferred Qualifications At least 6 months of experience related to health coaching. At least six months of direct experience in practice improvement. Demonstrated knowledge of practice improvement principles and methods. At least three months of experience designing and delivering interactive, skills-based trainings to healthcare professionals. At least six months of experience working in a primary care clinic. At least six months of experience using an electronic health record or chronic disease registry. Good analytical skills to understand how business needs can be addressed through the design and delivery of training programs.
