Job Summary
We are seeking a skilled and compassionate
Nurse Practitioner
to join our team at Lancaster General Health. As a key member of our primary care team, you will provide high-quality patient care and services to patients of all ages. Key Responsibilities
Provide Clinical Advice : Offer expert clinical advice to patients via phone or electronic correspondence, utilizing clinical judgment and triage protocols in accordance with regulatory requirements and standards of practice. Patient Education : Deliver patient-specific education on preventive care, chronic disease management, acute diagnoses, medications, and other areas pertinent to patient outcomes. Room Preparation : Prepare patients for visits by utilizing appropriate rooming standards tools. Medication Administration : Administer injections and medications as prescribed by providers and in accordance with department protocol, regulatory requirements, and standards of practice. Procedure Assistance : Prepare and assist providers with various procedures, minor surgeries, and other exams as designated by providers. Point of Care Testing : Perform Point of Care Testing, including but not limited to hearing, flu, urine, and strep tests. Specimen Collection : Collect blood or other laboratory specimens and prepare them for further testing. Prescription Refills : Authorize prescription refills as instructed by providers. Test Tracking : Assist with test tracking and monitoring overdue results. Wound Care : Remove sutures and change dressings on wounds as necessary. Exam Room Maintenance : Ensure exam rooms are adequately supplied and cleaned after each patient visit. Maintain and clean medical instruments and equipment after each procedure. Care Coordination : Assist providers and patients in reviewing and revising care plans and assess patient progress. Follow up with patients between visits as necessary. Proactive Outreach : Assist with proactive outreach for patients requiring preventive and chronic disease services. Referral Coordination : Assist in coordination of referrals, education, and community resources. Performance Improvement : Actively participate in performance improvement initiatives to improve overall patient experience and quality of care. Daily Huddles : Participate in daily huddles with providers. Patient Advocacy : Serve as a patient experience advocate by being a positive influence at all times when interacting with patients, visitors, and colleagues. Communicate with all patients, visitors, and colleagues in a professional manner while keeping customer service a top priority. Confidentiality : Handle all patient information in a confidential manner according to Lancaster General Health privacy and confidentiality policies and procedures. Secondary Responsibilities
Front Office Support : Perform various front office responsibilities as necessary. Regional Travel : May require some regional travel - valid drivers license is required if traveling. On-Call Responsibilities : Participate in the on-call responsibilities for this position as required. Other Duties : Perform other duties as assigned. Requirements
Education : Completion of a LPN or a RN educational program. Licensure : Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN), issued by the PA State Board of Nursing. Certification : AHA CPR/BLS certification required within 30 days of hire. Travel : This role requires occasional regional travel. Ability to travel to multiple sites is required. Preferred Qualifications
Nursing Experience : One (1) to Two (2) years of nursing experience in a primary or specialty care physicians office. Specialty Experience : Prior triage, emergency room, cardiac care, intensive care, or other related specialty nursing experience.
